2. FDA's public list of licensed cellular and gene therapy products is the checkable answer to 'is this FDA-approved?'. A product not on that list, and not being administered under an active IND, is not an approved therapy however it is described in a brochure. For orthopedics the list is short and its cartilage entry is MACI, indicated for focal full-thickness knee defects.

FDA states verbatim that regenerative medicine products including stem cells, stromal vascular fraction, umbilical cord blood, Wharton's jelly, amniotic fluid and exosomes have NOT been approved 'for the treatment of any orthopedic condition, such as osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain, or shoulder pain.' The only FDA-approved stem cell products in the United States are blood-forming (hematopoietic progenitor) cells derived from umbilical cord blood, approved only for disorders affecting blood production.

4. FDA's 6 December 2019 public safety notification followed multiple serious adverse events in Nebraska patients treated with unapproved products marketed as containing exosomes. FDA states there are currently no FDA-approved exosome products, that exosomes used to treat human disease are regulated as drugs and biologics subject to premarket review, and that clinics claiming otherwise are 'simply untrue' - naming clinics that also market violative stem cell products.

A CDC-led national public health investigation identified culture-confirmed bacterial infections in 20 patients (median age 63) across 8 US states who received umbilical cord blood-derived products marketed as stem cell treatment for pain, osteoarthritis, rheumatoid arthritis and injury. ALL BUT ONE REQUIRED HOSPITALISATION. Of unopened, undistributed product vials sampled, 65% (22 of 34) were contaminated with at least one of 16 bacterial species, mostly enteric; whole-genome sequencing linked an Arizona patient isolate to product administered in Florida.
